Pre-Appointment Essentials
To streamline your visit, please complete the new patient forms that will be sent to you via a secure link through text or email. These forms can be filled out conveniently on your mobile device or computer. If you encounter any issues accessing the forms, do not hesitate to contact us for assistance.
For those who prefer paper forms, please arrive 30 minutes prior to your appointment time. This ensures that we have everything prepared in advance for Dr. Leo and our clinical team.
Transferring Your Records
We are here to assist in transferring your dental records. If you would like our help, please follow the provided link to initiate the process. Keep in mind, it may take several days for records to be transferred from other offices, so we recommend starting this process early. We require all new patients to provide a full series of dental images, taken within the last two years, either during your visit or transferred beforehand.
Insurance and Payments
Unless notified 24 hours before your visit, we will assume you are opting for our New Patient Special, which includes X-rays, an exam and a healthy mouth cleaning for $189 or joining our Lafayette Smile Program. If you have insurance, please provide details at least 24 hours in advance to avoid unexpected costs, as verification can take time.
Appointment Policy
All new patient visits are scheduled for one hour and forty minutes. If you need to cancel or reschedule, please provide us with at least 48 hours’ notice to avoid a $100 rescheduling fee. This fee also applies if essential information such as records release, images or insurance details are not provided before your appointment.
Our Location
Find us off of 287 & Baseline, just behind King Soopers. For easy access, park on the west side of the building near the main ground entrance. We advise against parking under the large tree in the northwest corner of our lot for safety reasons.
